The UF Orthopaedics and Sports Medicine Insititute [1] (OSMI) treats a diverse nature of conditions. Our primary facility is located in Gainesville, Florida, adjacent to the west side of the University of Florida campus [2]. To save time, it is recommended to download/complete the patient registration forms [3] and bring them with you on your visit. If you are a referring office requesting consultation or transfer of care, download the consultation or transfer of care forms [3], and fax with all applicable notes. The OSMI physicians also provide quality patient care at other satellite locations in the North Central Florida area.
